Looking for Care Plan Referral Information?

Patients do not require a referral to see a dietitian unless they intend to claim Medicare rebates under a CDM/EPC or EDP plan prepared by a General Practitioner. However, we value communication with our patient's care team, such as a letter, to ensure a smooth transition of care. Please contact us to discuss how we can support your patient or make a referral.
